Charlotte Grange is part of Community Integrated Care, one of Britain’s biggest and most successful social care charities and has supported older people for over 3 decades. It is proud to deliver specialist residential and nursing care that supports people living with age-related needs and dementia to lead full and happy lives, through our EachStep Care model.

Charlotte Grange is a warm, welcoming and friendly home situated in the centre of Hartlepool. It is a purpose built 46 bedded residential care home which provides specialist care for people with age related conditions, including dementia. Charlotte Grange is within walking distance of local shops, restaurants, libraries and places of worship. We also enjoy excellent public transport links and are only a short distance away from the beautiful Hartlepool Marina.

Home Comforts

Charlotte Grange has some wonderful spaces to relax in and enjoy company of relatives and friends, including a potting shed view, a seaside view, a garden view and a view of the countryside. Our central lounge area provides a hub for the home with a sweet cart, hair salon area and cinema area. There are also lounges and dining areas for residents to relax. As a specialist dementia service, our furnishings and lighting are purpose designed to meet the needs of people we support.

We have accessible sensory gardens areas, so our residents can enjoy relaxation and activity outdoors.

We have been recognised for our person-centred approaches, winning the Putting People First Award at the Great North East Care Awards.

With our highly trained staff, specialist facilities and philosophy of promoting happy, active and fulfilled lives, we aspire to provide the highest standards of support.

Our doors are always open – there’s no restriction on visiting hours, however we do ask that to respect the dignity of our residents we would ask you avoid visiting during mealtimes.

Living Fulfilled Lives

At Charlotte Grange, we run an extensive activity programme and support people to enjoy days out in the community too. By getting to know our residents, we are able to offer activities that meet their different passions and interests.

Residents enjoy fun one-to-one and group activities, such as arts and crafts, reminiscence sessions, pamper days, gardening and cinema nights.

We hold regular social events for people we support and their families, including performances from local entertainers.

We welcome the contribution of family members. We have a Family Forum, who provide us with feedback and ideas. We work together to plan special days out and celebrations.

Food and Nutrition

Mealtimes are an important social event for our residents to look forward to everyday, and this begins with an appetising offering. We have a partnership with Apetito whose chefs and nutritionist have taken pride in catering for specific requirements and nutritional needs, without compromising on the delight of a delicious meal.

Apetito Chefs regularly update menus to keep things fresh, while also ensuring they’re filled with home comforts such as traditional Sunday roasts and fish Friday. Apetito also provide an award-winning Texture Modified range in line with IDDSI standards, allowing people we support who may have difficulty swallowing to enjoy their meal and dine with dignity. They also provide meals to special dietary requirements without compromising on taste.

Snacks and beverages are always available throughout the day and we even have snack stations located on each unit free of charge.

Person Centred Support

We are committed to providing personalised support that ensures our residents lead active, happy and fulfilled lives:

All staff receive specialist training and mentoring. We really get to know our residents and spend time understanding their life story and the things that are important to them, so we focus on the things that really matter to them.

Every resident has a named key worker who helps develop their own personalised care.

We hold a regular forum with residents, enabling them to voice their views on the day-to-day running of our home.
