We provide tenancy based rehabilitation/recovery services, for adults with mental health and learning disabilities. Our service users will be supported by both, in-house professionals; other local support networks, throughout the care pathway. We are uniquely placed to support people with a complex range of needs and work beyond traditional boundaries ascribed to health and social care services. Our main objectives are to assess the needs of potential service users employing person centred approach of care (bespoke support). Our input is delivered through an innovative model of care - Person Centred Planning (PCP) Model.

To prevent individual service user, relapsing and to prevent unnecessary admissions to hospital or locked facilities. Person Centred Planning (PCP) will ensure that service users are provided with living skills training, community support, wellness initiatives, healthy lifestyles support, advocacy and support with accessing local services for people with mental health needs, learning disabilities. In order to achieve our goals, we take seriously the safety of our service users, staff and the public we are committed to enabling risk management and staff development, training.
